Placenta Encapsulation Booking Form
Once your booking form is received, you will be issued an invoice. Once your payment has been received you will be sent a confirmation email with further information.
Terms & Conditions – I understand that Womb Service is providing a service, not a product. The service is processing your placenta into pill form for consumption by yourself at your own risk. This service is performed using fully sanitised and where possible, disposable equipment while wearing food grade gloves. It is my (the client) responsibility to ensure that my primary caregiver is aware of my decision to encapsulate and that my placenta is not disposed of. (Womb Service strongly recommends that your decision is included in your birth plan)
Refund policy - Womb Service limit the number of bookings in any month to ensure that commitments are met. In the case of change of mind or circumstances, a deposit amount of $70 is retained and the balance will be refunded. Should your placenta need to be sent to pathology or is deemed unsuitable to encapsulate, such as being covered with excessive meconium, then you will be refunded in full less a $25 admin fee.
The details provided in this form are private and confidential and will only be used for the purposes of processing your placenta. All answers are optional but your honesty is appreciated for the safety and wellbeing of yourself and any employees of Womb Service
